JSLPS SHG Rating Forms

A couple forms used by JSLPS to track SHG health. Thanks to @Falguni Ganguli (Unlicensed) for sharing
These formats are simply for internal monitoring purposes to get a sense of SHG/PG health.
These gradings are done on a sample basis.
There are no provisions for incentivizing SHG/PG in some way or other on the basis of these gradings, although, sometimes the SHG/PG is awarded with certificates or some mementos, no cash rewards or incentives are given.
JSLPS has no such plan for digitization of this thing, at least for SHGs (total number of SHgs under the JSLPS fold is more than 2.5 lakhs which is a pretty big number to catter), can be thought of for PGs.
To my sense, Jeevika also has such types of internal grading systems. I will check and get back asap.
